Enrollment Overview for Cottonwood Day School

Registration Requirements

• Completed Enrollment Packet: All forms must be filled out in full.
• Immunization Records: Each student must provide an up-to-date immunization record in compliance with state regulations or Indian Health Service standards.
• Tribal Membership or Indian Blood: Students must demonstrate membership in a federally recognized tribe or provide a Certificate of Indian Blood.
• Proof of Guardianship or Parentage: A birth certificate or other documentation establishing guardianship or parentage is required for enrollment.

What if my child has a medical issue or allergy?

Please inform the principal and registrar upon admission. They will ensure that appropriate staff are aware and necessary referrals are made to our Section 504 Coordinator.

Can students residing outside of the attendance boundaries enroll?

Yes, but they must obtain an out-of-boundary waiver approved by the school board.

Is there a deadline for transferring students?

Transfer students must enroll within the first 10 days of the fall or spring semester, subject to administrative approval.
